Henna Side Effects: Dangers and Contraindications
Pure henna most often causes minor discomfort — but one specific side effect, documented since the 1990s, deserves particular attention in infants.
Does pure henna (Lawsonia inermis) cause side effects? For the vast majority of users, no, or only minor, passing discomfort: slight dryness of the hair fiber, a lingering herbal smell, or occasional skin irritation. The most frequent side effects stay mild
- Dryness of the hair fiber: especially at the end of a long application, on already-dry hair, before softness returns after one or two washes.
- Lingering herbal smell: a few days, more or less noticeable depending on powder quality and rinsing.
- Temporary skin coloring: around the hairline or on the hands during application, harmless, fading within a few days.
- Mild scalp itching: rare, especially with a prolonged application or reactive skin.

Skin reactions: telling pure henna and black henna apart
A genuine contact allergy to pure henna remains possible but rare. The least-known side effect: hemolysis in G6PD-deficient infants
This is the most important point in this article, because it concerns a vulnerable population and remains largely unknown outside the medical field. Glucose-6-phosphate dehydrogenase (G6PD) deficiency is a common hereditary enzyme disorder around the Mediterranean, in Africa, the Middle East and Asia — the same enzyme whose deficiency causes favism, the well-known reaction to fava beans. In people with this deficiency, certain oxidants trigger accelerated destruction of red blood cells.
Now, lawsone, henna's natural pigment (2-hydroxy-1,4-naphthoquinone), is itself an oxidant. Zinkham and Oski, in a reference study published in 1996 in the journal Pediatrics, demonstrated in vitro that lawsone causes oxidation of hemoglobin and a drop in reduced glutathione in G6PD-deficient red blood cells, a mechanism consistent with several cases of unexplained neonatal hyperbilirubinemia that occurred after skin application of henna (see the study on Google Scholar). The same year, Kandil and colleagues described, in the Annals of Tropical Paediatrics, a series of fifteen G6PD-deficient newborns hospitalized in Kuwait for acute hemolysis after traditional henna was applied to the body, with anemia and hyperbilirubinemia significantly more pronounced than in controls (see the study on Google Scholar).
These two publications, since cited in many other case reports worldwide, converge on the same conclusion: in an infant whose G6PD status is unknown, applying henna — even pure — is not harmless. This is not a topic to downplay: untreated neonatal hemolysis can progress to severe jaundice requiring hospitalization, or even an exchange transfusion in the most severe cases reported in the literature.
